I'm not as much an expert on this as K1n9_Duk3, but: tl;dr: Keen doesn't use the EGA/VGA for timing, it uses the PIT. While it does try to update the screen during vblank to avoid tearing, this is not fed back into the game timing, so they can go out of sync. Timing in Keen 4–6 is quite complicated,...
