Some emulators and stuff also support "recording", with which you can record and later playback a file containing a series of timings and key inputs, and they hack that.
Sometimes they code their own tools, or use scripts, etcetera. I bet it's a bit more complicated for DOS games since this seems to be running from a virtual machine and not an actual emulator like DOSBox, though. Maybe a specifically programmed key press simulator?
